XDA Senior Member gamzekal ha posted an updated guide on porting the Xperia Z5 Small Apps to CyanogenMod 12 and 12.1. The guide can utilize either SystemUI from Xperia Z3 for compatibility on CM12 or use the SystemUI from Xperia Z5 for compatibility on CM12.1.
